The first form is an informational Welcome Letter. However, the second and third forms must be completed, signed, and returned to me at Paula@allswelltherapy.com before we meet for our first paid appointment. 

The content of these forms will help me fully understand your presenting issues and will help you fully understand the scope of my practice.
